Job description
Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients. Support patients and carers to achieve agreed outcomes and goals utilising the recovery model. Carry out complex risk assessment. Provide education, consultation, and advice to other care settings Apply a high level of understanding of the effect of mental health needs and provide training and advice on management of the condition, healthy lifestyles and well-being.
Apply in depth knowledge of any relevant health and social care Legislation and implications on intervention. Timely and accurate reporting and escalation of incidents and safeguarding concerns Maintain robust communication with the Older Adult Mental Health Service, across the Trust. Apply advanced communications skills to support patients with cognitive needs and advise on strategies for carers. Understand and analyse a range of information to assess and plan care for complex needs.
Ability to travel across the county Keep up to date with professional issues and developments in mental health and implement changes to practice in the light of this. Apply specialist skills and knowledge in order to establish ongoing professional competence. Maintain contemporaneous records across a range of information systems. Ability to risk assess and respond to crisis situations.
