RESPONSIBILITIES OF A MEDICAL SOCIAL WORKER SUPERVISOR: Working under the direction of the Director of Patient Care Services, and in collaboration with Interdisciplinary Team members, counsels patients and families regarding psychosocial adjustment to life threatening illness and long term disability. Provides information, referral, and consultative services to patients, families and staff; advises on appropriate utilization of community resources. Collaborates with other staff to provide a continuum of care.
ESSENTIAL FUCNTIONS OF A MEDICAL SOCIAL WORKER SUPERVISOR:
- Perform supervisory functions including but not limited to input for performance evaluations & supervised competency visits in field, mentor and provide direction/resources to MSW staff members, receive complaints and work with Director of Patient Care Services to resolve problems, assist in hiring/training of new employees, Coordinate MSW on-call calendar for the monthly schedule.
- Provide social service counseling, support, information and referral to patients and patient families regarding illness, impending death and related issues.
- Attend patient admission whenever possible or meet with patient and family soon after the admission to hospice services.
- Communicate with management and ancillary services; participate in community outreach.
- Provide consistent follow up on referrals as indicated.
- Maintain ongoing communication with Hospice team and attending physicians, when appropriate, regarding issues related to patient social services.
- Initiates and maintains a professional relationship with patients and families, demonstrating clear understanding of appropriate boundaries.
- Formulates an accurate diagnostic impression of patient and client strengths and weaknesses to facilitate social services planning; demonstrate an understanding of the nature of client resistance and respond appropriately to assure optimal patient and client care.
- Accept and work effectively with different ethnic, cultural, and language backgrounds; seek consultation when needed to resolve special problems. Demonstrate cultural sensitivity.
- Demonstrate ability to accept & work with a wide range of emotional and mental states; demonstrate understanding & effective use of principles & techniques of listening, interviewing & counseling
- Develop appropriate treatment plan for each patient and client; maintain records pertinent to patient care and client treatment; the expectation is to manage a case load of 15-20 patients.
- Document patient evaluation and chart each casework contact in patient electronic clinical records. Documentation to be completed within established agency guidelines and procedures.
- Develop information and referral resources and provide information and referral services to patients and clientele as needed. Maintain and update resource binder.
- Attend Interdisciplinary Team (IDT) meetings and participate in the development and modifications of the Plan of Care; provide direction and resources to MSW staff members and members of the IDT.
- Provide specific assignments and/or provide information for projects as directed by the Director of Patient Care Services and the Chief Executive Officer.
QUALIFICATIONS FOR MEDICAL SOCIAL WORKER:
- Masters in Social Work.
- 2 to 3 years post graduate work experience in the field of social work.
- Minimum of 2 years experience in Hospice Care or with experience in a health related field. (Home Health okay)
- Possesses and maintains current CPR certification.
- Willing to be flexible and help cover weekends.
Job Type: Full-time
Pay: $51.00 - $55.00 per hour
