The Medical Records Summariser summarises all records received at the surgery whether on paper or electronically, according to practice protocol. They will also ensure any relevant patient information received from external agencies eg hospital correspondence is accurately coded to the patient record.
In addition, the post holder will also support the multi-disciplinary team, providing administrative support as required, in line with the needs of the practice.
Key Responsibilities
The following are the core responsibilities of the Medical Records Summariser. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
· Weekly receipt and booking on to the EMIS system all medical records received
· Identify, at the time of booking records in, any which have not been received electronically. The Medical Records Summariser will then ensure these are requested, where relevant, either via the previous practice or Primary Care Support England
· Reviewing records and accurately recording a summary of the patient’s medical history and informing the GPs or other relevant members of the team of any concerns
· Accurately read coding data on EMIS
· Reviewing progress of summarising, ensuring practice targets are achieved
· Code all incoming correspondence in a timely manner
· Review all clinical letters forwarded to the Summariser ensuring they are processed appropriately in line with protocols
· File and store records as required
· Photocopy documentation as required
· Scan patient related documentation and attach scanned documents to patients’ healthcare records if required
· Input data into patients’ healthcare records as necessary
· Conduct system searches as requested
· Support all clinical staff with general administrative tasks as requested
· Review and update the practice summarising policy as required (in conjunction with the Practice Manager)
· Partake in audits as directed
The successful candidate will be educated to at least GCSE level and ideally have a clinical background. They will need to have experience of clinical coding and working in a healthcare setting. They will need to have excellent communication skills (written and oral) and excellent IT skills.
