JOB DESCRIPTION
JOB TITLE: Care Navigator
HOURS PER WEEK: 20 hours to be worked flexibly between 6.45am and 8.15pm Monday to Friday and may include some weekend working. You may be required to work additional hours to cover holiday, study leave and sickness absence.
It is important to note that there will be at least one evening shift per week.
LOCATION: New Hayesbank Surgery, Kennington, Ashford, Kent
JOB PURPOSE: To be the first point of contact between the patient, doctors and other members of the primary healthcare team and ensure that reception is fully equipped and runs efficiently.
To follow all practiceprocedures and protocols.
Support the practice in its aims within the Quality and Outcomes framework
Duties and Key Result Areas
Arrange appointments by telephone and face-to-face using the computerised appointment system.
Operation of the switchboard to include answering telephone queries and taking messages.
Take requests for home visits and ensure these are communicated to the doctor/nurse using the diary, day book and task message book.
Preparation of repeat prescriptions as directed by the doctor
Accurate completion of registration forms for new NHS patients, private patients and temporary residents.
Preparation of forms and other documents associated with patient services and ensuring that patients understand what is required of them.
Take payment for services provided and record fully in the receipt book
Reception of patients and direction of them to consulting rooms.
Ensure that tasks are completed or handed over effectively at the change of shift.
Maintain an awareness of health and safety and report any potential hazards to either the practice manager or assistant practice manager or in their absence a partner.
Act as chaperone when required by clinicians.
Preparation of consulting rooms and waiting area prior to surgeries and tidying rooms and waiting area after surgeries.
Ensure that reception is adequately stocked e.g. prescriptions, registration forms, prescription request forms and information leaflets.
Inputting, updating and extracting patient data from the practice computer system.
File papers in Lloyd George an envelope, ensuring all correspondence is in chronological order.
Assist with audits as required.
Liaise with the ambulance service under the direction of the clinician.
Attendance at training sessions and team meetings as required.
Assist in opening up and closing down the surgery. This is to include the following:
a) Switching computers on/off where necessary
b) Locking/unlocking all windows and doors
c) Arming/disarming the alarm
d) Locking away all prescriptions and other confidential material
e) Switching over telephone system to/from the out of hours service
Any other clerical task delegated by the Partners, Practice Manager, Assistant Practice Manager or Reception Manager and which is within the scope of the post holder.
Working Environment
Substantial periods of time on reception, on the telephone and on switchboard. Some time in front of a computer where high degrees of concentration are required and the ability to cope with interruptions.
The NHS is a constantly changing environment with increasing demands.
