Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
- Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
- Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
- Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Medical Records/Referrals Clerk

Job Title: Medical Records/Referrals Clerk
Reports To: Health Information Systems Manager
Area of Responsibility: Process requests for release of information from the medical chart in accordance with federal and state law and assists with administrative tasks in the clinic. Assists in all outbound specialist referrals as well as internal specialty referrals including scheduling, follow-up and documentation of referrals for clinic patients. Follow Patient Center Medical Home and Federal Tort Claim Act requirements to ensure patient safety.
Major Duties:
- Ensure timely processing of medical record requests from patients of CAPWN Health Center to other medical facilities.
- Ensure timely processing of requests for medical records from medical facilities, law enforcement, disability boards, attorneys, insurance companies, and Workers’ Compensation carriers, following privacy and legal requirements.
- Maintain the integrity and confidentiality of all Protected Health Information (PHI) and Electronic Protected Health Information (EPHI) documented in the CAPWN medical record and mandated by HIPAA standards.
- Assist with the purging of medical records following established policy and procedure.
- Scan and attach inbound records, identifying information that needs to be preloaded in patient’s medical record.
- Billing of patients and 3rd parties for copies of medical records.
- Review patient records with patients upon their request.
- Schedule, follow up and complete orders for referrals in a timely manner with external facilities as well as internal referrals.
- Maintain ongoing tracking and detailed documentation on referrals progress and status to ensure patient safety and strong customer service using Referral Tracking Tool.
- Assemble information concerning patient’s clinical history and referral needs. Per referral guidelines, provide appropriate clinical information to specialist and exchange information with outside entities to verify prior authorizations for procedures.
- Ensure that patient’s primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.
- Review details and expectations about the referral with patients, including financial assistance programs that are available at the referral site.
- Become knowledgeable of community and CAPWN resources, identifying and utilizing cultural resources.
- Establish and maintain relationships with identified service providers.
- Maintain a positive and professional attitude, striving to openly communicate with co-workers and patients, breaking down communication barriers when necessary.
- Play an active role in patient care using the provider led, team-based Patient-Centered Medical Home Care Model.
- Attend staff training sessions and/or meetings as required.

Minimum Qualifications:
Grade 10:
- Experience in a health information/medical records setting preferred.
- Strong interpersonal skills required in oral and written communication, time management, and critical thinking.
- High school diploma or GED.
- Above average customer service skills.
- Ability to work independently and unsupervised.
- Have a working knowledge of HIPAA Privacy Regulations, maintaining confidentiality in the provision of all services including written and/or oral reports.
- Working knowledge of Microsoft Office Suite, including Word, Excel, Outlook and Publisher.
- Current CPR certification or able to obtain within 3 months of hire.
Grade 11:
- Must meet minimum qualifications above;
- One year experience in a clinical/hospital health information setting processing requests of information (ROI) for medical records AND/OR
- Successful completion of Medical Terminology course or equivalent
Grade 12:
- Must meet minimum qualifications above;
- Associates Degree in Health Information Management (Technology) OR
- Current Basic Nursing Certification or Medical Assistant Certification OR
- Three years of experience in health care setting in the health information management field or referrals.
