Job description

ConcertoCare Medical Assistants deliver in-home and virtual clinical care support through hands-on collaboration with our clinicians at the bedside and direct engagement with our patients and their caregivers to ensure our patients get the right care, in the right place, by the right person, and at the right time. Key activities include support for in-home and virtual clinical visits, facilitation of virtual visits, the performance of health screenings and bedside clinical procedures, and virtual back-office support. The Medical Assistant performs some duties virtually and travels to provide care in the home alongside clinicians and facilitates virtual visits as scheduled. Medical Assistants are required to commute to the local office for training and meetings. This schedule will be set by the Practice Manager.

Our Medical Assistants:

  • Provide support at the bedside for in-home provider visits under the direction of the rendering clinical provider, including bedside scribe functions, measuring vital signs, administering health screenings, and maintaining up-to-date electronic health records for vaccination history, preventative care, and other quality measures.
  • Facilitate virtual visits at the patient's bedside to support care delivery under the direction of a remote clinical practitioner.
  • Provide support for clinical visits as directed including pre-visit chart prep, tracking results for diagnostic orders and referrals, managing quality measure orders and results, coordinating routine care scheduling needs, and ensuring appropriate follow-up.
  • Coordinate and support patient care by performing outbound calls and tasks at the direction of and under the supervision of a clinical practitioner.
  • Process and track orders and referrals placed by our providers as directed, working to ensure that patients complete orders and referrals in accordance with their treatment plan.
  • Deliver exceptional customer service to our patients, caregivers, and clinical partners including proactive, timely, and comprehensive communication from a holistic patient-centered perspective.
  • Clinical skills and procedures: Blood draws, laboratory specimen collection and preparation, diagnostic point-of-care testing, EKG, ABI, spirometry, basic hearing and vision tests, removal of sutures/staples, preventative and maintenance foot care, and medication and vaccination administration under the supervision of a clinical practitioner. All clinical skills require competency evaluation and are performed under the supervision of a clinical practitioner, with the scope of work and level of supervision as determined by state regulations.
  • Accurate and timely documentation in the medical record of all patient care support activities.
  • Promotion of and participation in patient engagement and experience initiatives and efforts.
  • Foster relationships with patients, caregivers, congregate living facility staff, and partnering organizations. Effectively coordinate patient care with caregivers and facility staff as needed.
  • Active participation in team huddles and organizational meetings as requested.
  • Adherence to compliance policies, procedures, and standards of conduct including all applicable laws and regulations.
  • Coverage for Care Navigator duties on an assigned basis, including front office administrative tasks.
  • Other market-specific duties as assigned.

Qualifications:

  • Current certification or registration as a Medical Assistant in the state where you will practice (in states that do not require Medical Assistants to be certified/registered, Concerto requires national certification)
  • 1+ year clinical experience in a healthcare setting required; urgent care, primary care, and community home health experience preferred.
  • Current certification in Basic life support (BLS) is required.
  • Phlebotomy certification or state authorization is preferred.
  • Ability to conduct patient home visits.
  • Prior experience caring for aging adults and/or adults with complex care needs is preferred.
  • Average to advanced computer and software skills are required.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on the vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No undisclosed violations and/or accidents within 3 years; No vehicle-related suspensions/reinstatements; No DUI, reckless driving, or felony conviction within the last 7 years.

Base Salary/ Wage Range $20.00 to $25.00 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
