Job Summary:

A vacancy has arisen for an enthusiastic, highly motivated Practice Nurse to join our group surgery.

The surgery consists of 16,000 patients, between two GP practices on a single site, in a centrally based location with a diverse population. The GPs are complimented by a large team that includes five Practice Nurses, two Health Care Support workers, and exceptional management and administrative support.

The successful candidate will be hardworking, adaptable, and reliable. We are looking for an enthusiastic and motivated individual, a dedicated team member who can work constructively with all members of the practice team.

Essential Skills: travel and baby vaccinations, cervical screening, health promotion advice, and the management of long-term chronic conditions. Work will involve all general treatment room duties, housekeeping, administration duties, and supporting the clinical team and GP’s.

Desired Skills:

Diploma in diabetes qualification or equivalent and ARTP Spirometry trained including interpretation of results.

JOB DESCRIPTION - PRACTICE NURSE

Job Summary:
Fforestfach Medical Centre is a friendly group practice, treating a diverse population. The nursing team consists of one Lead Nurse, four Practice Nurses and two Health Care
Assistants and aims to provide and maintain a high standard of nursing care to patients.
The duties include all tasks normally undertaken by an experienced RGN and any roles agreed between the partners and the nurses as appropriate, having regard to current training.

Job Responsibilities:

  • Assess, plan, implement and evaluate individual treatment plans for patients with known long-term conditions.
  • Proactively identify and manage treatment plans for patients at risk of developing long-term conditions.
  • Support Diabetes management clinics.
  • Undertake chronic disease management clinics.
  • Assess problems presented opportunistically by patients.
  • Minor illness consultations working to Practice protocols.
  • Travel immunisation and advice.
  • Offer advice and administer childhood and adult vaccinations under patient group directions according to national immunisation programmes.
  • Offer health promotion and lifestyle advice on diet, smoking, alcohol intake and exercise.
  • Perform ECGs
  • Perform ear irrigation
  • Perform cervical cytology smear tests
  • Perform venepuncture
  • Wound management and implement wound care in line with current evidence based guidelines.
  • Suture removal.
  • Assist in the provision of minor surgery.
  • Able to recognise and manage anaphylaxis according to current UK guidelines.
  • Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.

  • Support the Practice clinical governance agenda.
  • Ensure effective relationships and communication with the whole Primary Health

Care Team and other agencies

  • Identify changes to clinical practice that are required to implement evidence-based guidelines.
  • Support others with their training and development needs.
  • Manage and organise individual patient consultations.
  • Attend staff meetings.
  • Partake in administrative duties as per the needs of the practice and patient recall and monitoring.
