About the Role
Dr Laliwala and partners are looking to welcome a Practice Nurse to join the team on a permanent basis.
The main responsibilities of the role include:
- Assess the needs of patients and deliver appropriate treatment, screening and advice
- To manage care for individuals within specific groups
- To identify, manage and support patients with or at risk of developing long-term conditions and implement individual treatment plans
- Carry out preventative screening
- Implement vaccination programmes for adults and children
- Participate in activities relating to QOF
- To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance
- Develop, review and implement nursing protocols in conjunction with others
- To request pathology services and process pathology results as required
- To maintain accurate clinical records in conjunction with extant legislation and ensure read codes are used effectively
- Prepare for CQC inspections
- To support the team in dealing with clinical emergencies
About you
As a key member of our team of Doctors, Physician Associates, Nurse Practitioners, Clinical Pharmacists, First Contact Physiotherapists, Social prescribers, Nurses and HCA’s you will be:
- An NMC and RGN registered nurse, with an up to date PIN number renewed annually
- Able to work autonomously
- Ideally you will have experience of working in primary care and have the following clinical skills:
Wound care
ECG’s
Venepuncture
Women’s Health
Immunisations
Chronic disease management
- Excellent communication skills, both written and oral
- Good interpersonal skills, committed to supporting the team and the ability to work under pressure
- Effective time management
- A commitment to ongoing professional development
