Caseworker Specialist II
The Division of Services for People with Disabilities, within the Department of Health & Human Services, is looking for a dedicated professional to fill our Caseworker Specialist II position. As the Caseworker Specialist II you will be doing face to face assessments, intake and eligibility reviews for those seeking or receiving DSPD services; managing individuals on a waitlist, as well as providing resources and referral for families in need of additional outside services while waiting for DSPD services. This is a great position to assist the families of Utah, and also has the potential for teleworking.
This remote/telework position requires work to be performed anywhere within the State of Utah. Requires some travel throughout the state to various Department of Health & Human Services (DHHS) locations.

As the Caseworker II Specialist you will:
- Conduct eligibility reviews for those applying for DHHS services.
- Transition individuals from wait list into services.
- Coordinate and refer services of community resources, placement providers, collateral agencies, the court, and/or other community partners with families, clients, or patients receiving services.
- Update annually the Needs Assessment Questionnaire and Waitlist Survey
- Has knowledge of Home and Community Based Services (HCBS).
- Is passionate about empowering People with Disabilities.
- Is an excellent listener who is also very proactive.
- Has knowledge of community resources for people with disabilities and how to access those resources.
- Has the ability to work well with a wide range of people with varying interests.
- Must be able to observe physical conditions, behavior and surroundings and listen for sounds that may indicate potential areas of concern or inappropriate activity.
- Must also be able to observe clients for potential concerns.
- Risks found in potentially dangerous or unusual environmental stress situations, e.g., being subject to possible physical or verbal attack.
- Work requires physical exertion. May require the ability to stand; walk over rough surfaces; bend, crouch, stoop, stretch, reach, or similar activities; may require lifting of moderately heavy items (up to 50 lbs.) such as computers and record boxes.
