Job description
You will be responsible for assisting with the delivery of patient centred care under the supervision of a senior member of the South East Home Treatment Team; providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the Team.
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required.
In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.

Core clinical functions
Assist in the management and care of service users under supervision
- Produce accurate and timely documentation / records on all clients in line with relevant codes and Trust policy
- Assist senior staff in the ongoing assessment and observation of service users and provide timely and appropriate feedback
- Assist in the risk assessment of the individual and of the potential hazards in the SU environment
- Provide practical support to service users, carers
- Transport / accompany service users to appointments
- Attend Team meetings i.e. referral / review / business as appropriate
- Report and document incidents as required
- Support service users in activities of daily living such as meal planning, shopping, budget management
- Support with employment, housing and benefit claims and paperwork
- Undertake physical health care checks of service users, document and report results
- Under supervision take part in assessing and reviewing care
- Under supervision take part in group work (low level) – such as relaxation, health promotion, sleep hygiene etc

- Complete mandatory training in line with the Trust policy
