Job description
Our client D is a 31 year old man who has a traumatic brain injury. He has a great sense of humour and is very sociable, he loves music, films and TV and football (especially Manchester City) trips out and visiting with family.
Currently he resides in a nursing home and he requires support to work with the team in the nursing home and also facilitate greater socialisation and community access.
D has an established MDT who would offer the necessary training and support to enable the support worker to carry out therapy programmes during their working hours.
D is non-verbal and is learning to use assistive technology to facilitate this, and full training would be provided in the use of this equipment. Someone with previous experience of using assistive technology would be advantageous.
D is a full-time wheelchair user and has his own wheelchair accessible vehicle to allow him to access his local and wider community.
A private case manager is involved, working alongside the client and family. A full induction training programme will be organised in order to familiarise the successful candidate with our client’s needs and complete training on his therapy programmes.
A support worker is required for 24 hours per week, however, would need to be flexible with days and times to accommodate the client’s schedule. The candidate would not be required to work every weekend.
Key Responsibilities
- To promote the client’s well-being and happiness. To provide assistance with aspects of the client’s ongoing medical care and rehabilitation under the supervision of treating doctors and therapists.
- To assist the client in accessing his local and wider community and attend all scheduled appointments
- To plan weekly activities and be flexible and adaptable to the client’s changing presentation
- To maintain a safe environment for the client, in the home and in all community-based activities (subject to completion of risk assessment training).
- To follow the support programme agreed with the case manager, therapists and the client.
- To advise the family, or where appropriate the case manager of any concerns regarding the client’s physical and psychological health.
- To use initiative when unforeseen events occur, and the family / case manager are not directly available to provide guidance.
- To work as an individual responsible to the case manager and to ensure good communication, flexibility, and a consistent approach to care.
- To attend support worker meetings and training sessions as requested.
- To make a daily report following each duty regarding the client’s welfare.
- To liaise with the case manager for annual leave requests and alert the case manager of sickness that will prevent them working, in order that cover can be organised.
- To report to the case manager regarding general progress and any incidents that may raise concern
- To respect the privacy of the client. All matters relating to the client’s situation and family are to be treated as confidential and are not to be disclosed to any third party.
Qualifications/Experience
- A full clean driving license is essential for this post.
- You should have a diploma in Health and Social Care level 2/NVQ or equivalent.
- The successful candidate will require 2 references and a full enhanced DBS check will be completed following acceptance of this position.
- Previous experience of working is a support environment for 1 year (preferred)

The rates of pay are £15.00 per hour week days and £16.00 per hour weekends.
