Are you a people person that likes to encourage others to share their life story? If yes, you could be part of the JCR In Reach service at the Conquest Hospital in Hastings! The JCR In Reach service is a new exciting element to JCR where you are identifying suitable clients to return home with JCR Reablement. JCR Reablement is a service that supports individuals to regain their independence to remain safely at home. The client will have goals to achieve at home over a short time-frame and JCR supports rehabilitating the client to ideally get back to how they were before their hospital admission so they can live their life as they wish. Your role is to: Identify suitable clients ready to go home with JCR. Be able to communicate with patients regarding their wants, wishes, goals and outcomes. Work in a fast-paced environment. Stay calm and liaise professionally with others. We’re looking for candidates who are: Organised. Note: The working pattern of the role is predominantly 8:30am-5:00pm Monday to Friday but you will be part of a rota where occasional working at the weekend is required. Weekend shifts will be a 6 hour shift between the hours of 7am and 1pm or 4pm-10pm. You will receive up to two additional salary increments for weekend working, equating to a salary of up to £23,620 - £25,271 per annum. This role is not eligible for sponsorship.
