Job description
Processing referrals to community for discharge Answering the telephone and dealing with enquiries from wards and family members in a polite and professional manner Maintaining a variety of spreadsheets and IT systems ensuring data is accurately recorded Liaising with wards and external partners to inform them of discharges and to problem solve when required Escalate issues to Team Leads Attend meetings and record actions To work as part of a team sharing responsibility for the smooth running of the service.
