Permanent Contract | £14,565 to £15,961 per annum (Scale F) | 22 hours per week | Reed House, Norwich
Central Norfolk HomeFirst Hub An exciting opportunity has arisen within the Central Norfolk HomeFirst Hub Integrated Care Coordinator (ICC) team for a part time ICC on a permanent contract.
Working on a 2-week working pattern, this post will include weekend working. Norfolk has a long-standing commitment to ICC roles, seeing them as essential to the development of integrated teams and ways of working. This role will see you working for the Central Norfolk HomeFirst Hub which covers Norwich as well as North and South Norfolk. The HomeFirst Hub is an integrated team working closely with Norfolk Community Health & Care (NCH&C), NHS Acute colleagues, Voluntary sector and other Norfolk County Council colleagues supporting individuals on the Discharge to Assess pathway home and those experiencing complex admission avoidance situations.
You will plan and coordinate discharges from community and acute hospital settings to allow individuals to return home, access reablement opportunities and plan their ongoing care and support needs once discharged. The model also includes supporting people in crisis already in their usual place of residence and preventing unnecessary admissions to hospitals or other settings such as care homes. You will work alongside many different professionals and services such as Nurses, Therapists, Care providers and Social Workers, fostering and promoting multi-agency working.
You will be positive, flexible and open to change which is sometimes fast paced. You will link closely with all other ICC’s within the team and support the wider team demands where needed and will work specifically closely with colleagues in community and acute settings as you support the development of the discharge and admission avoidance pathways. You will monitor and track the patient journey and support colleagues to manage complex situations using data, system knowledge and onward referral pathways. You will work with multiple systems such as LAS, SystmOne, PAS and NCAN and will support referrals to voluntary organisations
. You will be a highly motivated and enthusiastic candidate and will have a high level of education (eg NVQ level 3, GCSE Levels or equivalent) and previous experience of working in voluntary sector, primary care, the NHS or Social Care. A positive can-do attitude is essential alongside professional curiosity around information exchange and community resources. You will have an awareness and understanding of relevant health and social care legislation, excellent interpersonal and organisational skills along with a high level of administrative and IT skills. You will be competent at working at pace in a responsive fast paced environment.
The HomeFirst Hub operates a blended model of office base and virtual working, currently from a range of bases including the Norwich Community hospital and Broadland Business Park. We are looking for a longer-term base to continue to develop this dynamic integrated team. We offer a friendly, supportive team, regular supervision, and the opportunity to develop your skills and co-produce the role in line with the NHS Long Term plan.
For an informal discussion about the post and working hours please contact Megan Anderson - ICC Team Leader for HomeFirst Hub (Central Belt) on 01603 222872.
To optimise the experience for our service-users/patients, we are further developing our hospital pathway.
