About this role:
Southampton City Council , Social Wellbeing service are looking for an experienced person who is enthusiastic about working with people who live in Southampton and have care and support needs. Prior experience of working in care or support roles will be an advantage. In the Social Wellbeing service the approach is to work in an enabling, person centred way that’s builds on the individuals strengths and takes account of the often significant challenges that they encounter. You will have strong communication, assessment and recording skills. The successful candidate will have the personal attributes that will enable them to work collaboratively along side the person ,their family and carers. This will ensure that the persons views are at the centre of how their care and support is arranged. The Review and Flexible Response Team is a supportive team that works together to ensure high quality assessments are undertaken. You will receive regular supervision and support. You will be encouraged to access the SCC comprehensive training calendar and other learning events in order to support your development and learning. There are career development opportunities and many Independence Advisors go onto access an Apprenticeship in Social Work.
The main duty or the role is to complete annual care reviews ,this will be in the persons home, remotely or in residential care settings. You will work in a fast paced environment, completing high quality assessment in a timely way.
Southampton City Council Operate a flexible working policy, there is an expectation that you will be able to work from home and also from the office.
You will be operating with a high degree of autonomy and managing you own workload and diary. You will be instrumental in ensuring that at the point of review that the person is in receipt of high quality care that meets their needs . We are looking for an independence advisor who has experience and knowledge in the following areas
- Knowledge of the Care Act Assessment framework and eligibility is desirable
- Ability to identify and report adult safeguarding concerns
- Able to identify community resources that can meet need as part of the persons care and support plan.
- An understanding of the concept of Mental Capacity and Best Interest is desirable
- Producing best value care and support plan that include community resources where appropriate .

- This post requires an Enhanced DBS check.

- Please note the salary quoted is based on full time hours (37 hours).
