This is an exciting new opportunity to work as part of an in-reach team, working closely with the community inpatient units to support appropriate, safe, and timely discharges from the acute to our community beds.
The role will be based initially in the Royal Berkshire Hospital, and you will be working with the wards to ensure the referrals are suitable for a community hospital, have the correct information for a safe discharge and to support the acute wards with their understanding of the patients accepted to a community hospital bed.
The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

The In Reach Coordinator will assist in facilitating a discharge from secondary care, working closely with the In Reach Team, Intermediate Care, Community Health services, Primary Care and Mental Health services to ensure robust proactive care planning.
The In Reach Coordinator will be a member of the In Reach Team and will be the point of contact for the In Reach Team and other professionals within the Community, local acute and community hospitals to manage the safe transfer of patients from the Emergency Department and acute wards back into the community.
They will coordinate the daily running of In Reach Team. This will include liaising with a number of stakeholders to ensure measures are in place to facilitate a safe discharge and have an overview and understanding of the capacity of the teams and the community beds in order to improve the flow into the community services helping to reduce delays and hospital length of stay.
They will also be involved in difficult and complex conversations regarding capacity with service users and stakeholders, including senior clinicians in a professional manner, responsible for delivering bad news if demand outweighs capacity.

- NVQ4 or NVQ3 or recognised certificate in office administration or equivalent experience.
- Experience of working in a variety of settings (hospital and community)
- Experience of using patient databases e.g., RiO, EMIS, Adastra, IBIS and robust information sharing between them
- Experience of multi-professional working including MDT care planning
