A kind and compassionate Social Worker to work within the Intermediate Care Allocation Team for Adult Services at Lancashire County Council.

This is a wonderful chance to make a real difference to the lives of vulnerable individuals within the community who require much needed care and support. We seek to promote independence and well-being at every opportunity, giving our service users the strength and courage to lead normal and healthy lives once again. As a Social Worker experienced in working in Intermediate Care settings, you will be reaching out to those in crisis or who are acutely unwell within the community and the hospital itself. We receive many compliments as a service from colleagues and service users who have benefitted from our intervention, so you will be joining a "team to be proud of" and strive to continue with our superb standards.

There is an expectation for you to travel to different locations to provide assessments therefore a driving licence and access to your own vehicle is essential. As this is a 7-day service there will be a requirement for you to work 1 in 4 weekends, we offer pay enhancements for weekend working.

Challenge – the world of social care is never without its challenges and this environment is no different with the variety of work we have on offer – there is always a fluctuation in demand so we need a professional who is self-motivated and can take a proactive approach to ever changing priorities – from offering advice to NHS colleagues and non-social work professions to dealing with bed based requests, you will be supporting with an amalgamation of varying responsibilities – it is imperative that our service users remain at the heart of everything you do and that we always endeavour to achieve the best possible outcomes for them

You must be registered (SWE) to be appointed as a Social Worker or have applied to register with the Social Care Council. For Social Worker/Care Manager positions where non social worker qualifications are accepted, registration is not required, and you will be appointed as a Care Manager.
