My Space Housing Solutions provides exempt accommodation to some of the most vulnerable people in society, supporting and enabling them to live independently.

Our approach means vulnerable people can live in good homes with the necessary support, but also puts them on the path to improving their wellbeing, integrating with their communities and living independently.

What you’ll be doing …

· Taking the lead role in the assessment of incoming referrals.

· Working with individuals in a focused way which enables them to maximise their independence and live to full potential

· To work alongside vulnerable individuals in promoting independence and empowering sustainment of their tenancy.

· To complete a support plan with each tenant detailing all elements of support required in order for their tenancy to be maintained – including any signposting required. This must be reviewed on a regular basis.

· To work alongside an individual in understanding how best to provide support through a thorough risk assessment process to highlight any risks to themselves, their tenancy or others – this must be reviewed on a regular basis.

· To provide information, advice and guidance to enable tenants to understand their responsibilities and support them to develop effective strategies to manage those responsibilities.

· To work closely with other professionals where appropriate in order to provide a holistic approach to sustaining the tenancy including coordinating professionals’ meetings when necessary and attending external meetings in relation to their support needs.

· To liaise internally with all other departments to enable the successful cycle of tenancy related support and to help maintain the tenancy.

· To work closely with an individual in understanding acceptable behaviour and being a good neighbour so that their tenancy is maintained in the longer term.

· To use the recording systems in place to enable a full reflection of all activities relating to each individual whether it be contact notes, all relevant supporting documentation, maintenance reporting etc.

· To be responsible for a series of regular Health and Safety checks effective recording and reporting any compliance related matters

· To be responsible for regular property inspections, reporting maintenance and other issues as required

What we will be looking for from you …

· A confident approach with exceptional verbal communication and negotiation skills

· An appetite to work collaboratively with colleagues to problem solve and work as part of a team.

· Excellent written and recording skills

· The necessary skills and abilities to complete comprehensive assessments at initial referral, support plans, risk assessments and acceptable behaviour agreements.

· Ability to prioritise a busy and ever changing workload

· A solid working knowledge of safeguarding adults and children, as well as experience of working with a diverse group of vulnerable adults

· The confidence and experience to challenge decisions and advocate in the best interest individuals

· Car driver with access to a car for business use.

Salary: £20,800.00 per year
