Job Title: Homeless Transition Worker
Reports to: Youth Service Manager
Salary: Scale 6, Points 26-28, £24,744 - £26,398 per annum, pro rata
Hours of work: Part Time (18.75 hours per week)
Key Responsibilities:
- To provide an effective Homeless Transition service within the Birmingham Youth Hub to young people moving to non-commissioned accommodation.
- To provide a prompt and proactive, non-judgmental response to referrals to ensure engagement from the young person.
- To provide flexible, person – centered support and settlement assistance to identified young people.
- To follow the principles of No Wrong Door Network to ensure that all young people are actively encouraged to engage and access the correct support.
- Ensuring effective follow up and monitoring of case load.
- To develop and complete a transition checklist for young people as part of the move in process.
- To undertake appropriate background checking to ascertain level of risk and need i.e. previous landlord, parent/carer, probation etc.
- To work in partnership with the local housing services and other referring agencies to prevent a young person becoming homeless.
- To provide effective telephone advice and guidance to customers where required.
- To ensure clear and professional case records are kept up to date and accurate at all times.
- To work closely with other organisations and agencies to provide holistic multi agency responses to the housing and support needs of young people.
- To identify and develop a wide range of joint working relationships with organisations working to improve quality standards across a wide range of housing options, incl. BCC, BVSC, benefits and revenues, PRS.
- To develop and maintain effective partnership arrangements with regulated and/or approved Private Landlords, Social Housing Providers and Supported Accommodation Providers through effective communication and clear understanding of roles and responsibilities.
- To undertake external visits such as property checks, home visits and accompany young people to appointments, where appropriate.
- To provide specialist housing advice, including where appropriate referrals to legal advice services incl. NHAS.
- To advocate on issues such as benefits and housing rights, alongside ensuring clear expectations of the tenant and landlord.
- To maintain up to date relevant information and resources related to housing and housing related support.
To operate within a duty system at the Youth Hub.

QUALIFICATIONS
- NVQ 3 Advice & Guidance, Health and Social Care or equivalent or working towards Youth Work, Social Work, or equivalent
ROLE EXPERIENCE
- Working with young people in the following settings/
- Types of work – Youth Work, Residential or Social Work, Probation, Community Work, Education and Training, Housing.
- Working with Volunteers/Students
KNOWLEDGE
- Housing Rights, Welfare Reform Knowledge
- Relevant knowledge relating to homelessness, incl. Homelessness Reduction Act 2017
- Knowledge of Children’s Act i.e. Section 17, 20 & 47
- Knowledge of Safeguarding procedures and legislation, i.e. Care Act 2014
- Knowledge of and Commitment to E.O.P/Anti-Discriminatory practice/ethical practice
- Knowledge of Social Skills/Job Training skills.
- Knowledge of local services and agencies relevant to young people in the city
- Knowledge of the exempt sector in Birmingham
