Fairview Range is looking for a Home Health Aide (HHA) to join our Hibbing Home Care and Hospice team. We consider it an honor to provide care for our patients and their families, and serve communities across the Iron Range. The HHA works under the direction of and in collaboration with the primary nurse to provide care to patients in their homes. This position is part time, approximately 50 hours per two weeks, 6-hour Day shifts. This position will primarily be providing care in the Hibbing and surrounding area; flex locations Virginia/Eveleth area. Fully benefit eligible!

Responsibilities/Job Description:- Assists with the delivery of age specific direct care to patients in the home setting under the supervision of the Home Care Nurse.
- Provides personal care and performs other delegated duties as assigned by the RN or therapist such as skin care, assistance with a home exercise program and assistance with self-administered medication.
- Light housekeeping related to personal cares.
- Documents all services performed.
- Communicates abnormal patient findings, change in condition or incidents to the RN Case Manager or Supervisor.
- Must be at least 18 years old.
- Successful completion of a nursing assistant training program, Home Health Aide course or its equivalent and/or successful completion of competency test.
- Current MN driver’s license and insurance.
