We are looking for a Health Care Support Worker to join our dynamic team, based out of The Oak Tree Centre in Huntingdon and The Peacock Centre in Cambridge. The team cover the whole of Cambridgeshire delivering quality care to make a difference to children and young people requiring nursing care in their homes or other community settings. We have a dynamic team who facilitate early discharge of children from hospital as well as prevent hospital admissions and ED attendance, and are looking for enthusiastic, passionate and motivated Health Care support worker to join our team. To work within the Children’s Community Specialist Nursing Team in delivering care to children with acute and complex healthcare needs in the community. This may also include children who are nearing the end of their life. To deliver specific nursing interventions, promote transition to adult services and social and play tasks. To carry out duties to ensure the smooth running of Community Paediatrician clinics in the child development centre. The post-holder will also be expected to undertake some clerical duties. To act under direction of qualified professionals. Work alongside other staff to provide both a quality and child friendly service. To work within the multidisciplinary Children's Community Specialist Nursing Service under the supervision of the senior case holders in order to provide individualised care to children and young people with complex and acute health needs, as specified by the child’s protocols and in line with trust policies. 2. To provide care in the community to children and young people with complex and acute health needs for which training and competency assessment will be required. This may include – All procedures relating to naso-gastric feeding and care All procedures relating to gastrostomy/jejunostomy feeding and care Venepuncture/ Fingerprick bloods Wounds and dressings 3. To deliver care to patients, carers and their families adhering to the philosophy of family centered care. 4. To use a range of verbal and non-verbal communication tools to communicate effectively with patients and colleagues to progress their care. This may include patients who have cognitive impairment or difficulty communicating. 5. To act as a chaperone for vulnerable children and young people as per Chaperone policy. 6. Promote the health, comfort, dignity, privacy, and quality of life for children and young people. To empathise with the patient and carer. To be sensitive to individual patient needs, expectations, and cultural beliefs. 7. To work in conjunction with the Paediatrician to promote and facilitate transitions to adult services. 8. To provide distraction and assist in helping children and young people with their coping strategies undergoing any procedures that they may find distressing. 9. Keeping the consultation and assessment rooms and other areas used by the children, young people and their families, or used by the staff on behalf of the patient, clean and orderly. 10. Ensure first line resuscitation equipment maintained in line with trust policy. 11. Assist in maintaining a clean and safe environment. 12. To carry out administrative duties as required e.g. checking stocks, ordering. photocopying / faxing / documentation. 13. To ensure that all duties are carried out to the highest possible standard.
