As a Community Healthcare Assistant at the Virtual Ward you will be expected to work with the wider MDT team and support the day to day caseload for admission, review and discharge. Troubleshoot, safety net and make clinical decisions to keep patients safe at home with the support of your clinical leads.
Please note this vacancy role requires face to face patient management as well as virtual management and therefore the team members will be expected to conduct face to face assessments in line with patient/ caseload needs.
Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
Be required to use tact and persuasive skills in order to gain the patient’s co-operation in their care management plan.
Be aware of assistive technology and utilise where appropriate.ie (Luscii, TekiHealth, Docobo).
The post holder will be expected to utilise technology to support the role, including undertaking online and video consultations with patients.
Ensure patient held records are completed for each point of contact and inputting escalation on Clinical Web Portal on a daily basis.
Please note this role involves remote monitoring of patients as well as face to face visits in the patients home.

Overview of the role will be the following:
Virtual ward software monitoring of patients
Identifying and proactively onboarding patients on the virtual ward caseload
To work as part of a team and the wider MDT
Good wide range of assessment skills
Teaching and inducting of new starters and students
Being part of n MDT meetings
Support with projects and service improvement
Develop long term condition management skills and knowledge
Face to face contact with patients to conduct initial assessment and base line observations.
Being organized and a role model to others
