The Oldham Community Diabetes service is a multidisciplinary team that provides specialist diabetes care within a community setting closer to patients homes. The service also provides education sessions to patients, GP’s and primary care clinicians.

An opportunity has arisen for an enthusiastic and motivated healthcare assistant with excellent communication skills to join the Diabetes Team as a Band 3 Health Care Assistant.
You will work closely with the Consultants, Clinical Nurse Specialist’s and other multidisciplinary team members playing an important role in the delivery of effective care and management of patients with Diabetes in a clinical setting or patients own home.
Education to Patient’s on blood glucose monitoring will also form part of this role.
Opportunities for further development and training within the speciality may be available.
The work will take place in a variety of community settings including patients homes, nursing/residential homes and intermediate care units.
The post holder will often work in the community as a lone worker, in line with organisational lone worker policy.
Main duties of the job
You must be able to work autonomously and organise your workload effectively.
You will need to be flexible and will be required to work within a variety of clinic locations as well as home visits within the Oldham borough.
You should hold an NVQ level 2 qualification in healthcare
Proficient in Phlebotomy
Good communication skills

Essential Qualifications
Essential criteria
- A good standard of general education
Desirable criteria
- NVQ Level 3 in Health and Social Care or equivalent qualification
- Basic Life Support Course
Knowledge, Skills and Experience
Essential criteria
- Previous healthcare experience
- Good communication skills (written and verbal)
- Understanding of how illness affects individuals and their cares/families
- Ability to work as part of a team
- Willingness to work flexible shifts
Desirable criteria
- Evidence of the acquisition of relevant skills associated with the role i.e venepuncture
