Job description
An experienced Band 3 HCA to work autonomously visiting cardiac patients in their own homes. The job is to support, motivate and deliver an individualised exercise programme, that has been assessed and prescribed by our Band 7 Physiotherapist. The patients are seen by our experienced Community Cardiology Nursing Team prior to them being referred to the home exercise programme. The patients have either Heart Failure, or had a recent acute cardiac event or heart surgery. We are looking for a very enthusiastic, confident nurse to join our dynamic innovative rehabilitation Team.
