Job description

Within the Amputee Specialist rehabilitation at NCH&C, we are looking to recruit a Health Care Assistant into a permanent full time post. If you are looking for a change we may be the ideal place for you! We are a small friendly team who use our specialist skills in caring for patients who have had a amputation from across the region on our 9 bed inpatient ward, not far from the Centre of Norwich.

You will work as an integral part of the ward nursing team and be responsible for providing high standards of rehabilitation care to a group of patients with rehabilitation needs. This post involves working a combination of early, late and night shifts covering some weekends and bank holidays.

You will deliver evidence based care to a high standard. Excellent communication skills and a supportive approach to team working within a rehabilitation setting are essential to the success of this service. You will demonstrate the ability to working within the Trust's behavioural framework which will be essential to the role.

An ability to manage your own learning supported by the team gaining new knowledge and skills to benefit yourself and the service will be vital. Professionalism is an important part of the role and we would expect staff to be able to deliver this.

Good interpersonal skills are essential as is the need to be cheerful and courteous at all times. Good computer skills or the ability to learn will be required for this role is this is essential to this unit. Record keeping will also be essential part of the role and important to ensure high standards of care delivery for this post.

  • Implement care for patients on the unit as defined by the individual care plan.
  • Provide a high standard of direct care for patients in line with the ward philosophy.
  • To carry out direct patient care in line with ability and job definition.
  • To establish relationships with patients, family and friends that helps allow them to
express their needs.
  • Communicate relevant information regarding the needs of patients, family and friends to the registered nurses.
  • To undertake regular reviews of patient care with the registered nurses.
  • To undergo development and review appraisal annually or more frequently if appropriate.
  • Take responsibility for keeping up to date with statutory training in line with Trust policy.
  • To be able to demonstrate a knowledge of the limitations of own practice and competencies.
  • To record care delivered and or communicated in the patients record.
  • To assist with maintaining the ward environment in a manner that promotes the therapeutic and health needs of patients, carers and visitors.
  • When appropriate, to socialise and interact with patients, family and friends.
  • When appropriate, to accompany patients on leisure activities in accordance with their therapeutic needs.
  • Working closely with the therapy teams to ensure multidisciplinary approach to care is delivered.
  • To ensure the safety of patients, visitors and staff and report all incidents/hazards to theward manager.
  • To ensure the care and custody of patients property and valuables.
  • Observe the confidentiality of patient’s information at all times, in accordance with the
Data Protection Act.
  • Undertake any other duties commensurate with the grade.
  • To help maintain a high standard of communication between staff on the unit.
  • To attend ward meetings as a member of the ward team.
  • To greet visitors and staff entering the ward ensuring that they are directed appropriately.
  • To communicate appropriately to patients using the care plan/speech therapist as
guidance.
  • To work collaboratively as an integral part to the multidisciplinary team.
  • To contribute actively to the rehabilitation process.
  • To ensure the detailed understanding of patients and needs and abilities gained through
the skills and observations of the nursing team are brought to the goal planning process.
