Job description

An exciting opportunity has arisen within ICRAS Southport and Formby; we are seeking to appoint a highly enthusiastic and motivated individual to join the service as a Health Care Assistant. Successful applicant would work under the direction of the ICRAS nurse practitioners supporting the provision of high quality holistic care to patients. The Health Care Assistant will work without direct supervision providing care to patients in their own homes and other community settings, as delegated by the Nurse Practitioners following appropriate training and supervision to ensure clinical competence and safe effective care.

Operational hours of the service are 24 hours a day 7 days per week 365 days per year

The post holder will work under supervision as part of the multi-disciplinary team and will assist and support the clinical team(s). This may include working within the patient’s home environment and other clinical settings in the community.

Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

Work as a member of the multi-disciplinary team.

Undertake a range of patient interventions within the scope of NVQ level 3 skills for patients as delegated by service lead or other qualified clinical staff.

Offer advice to patients and carers on a wide range of health issues to include health promotion, lifestyle advice and self-care.

Plan and organise own work schedule once delegated from service lead.

Undertake basic patient observations.

Work within existing clinical guidelines, policies and infection control procedures.

Offer advice and support to patients and families in a wide range of situations.

Communicate factual and sensitive information within confidentiality guidelines to patients, ensuring all information is appropriately documented and reported back to the service lead.

Demonstrate own role to new starters and students.

Communicate information to multi-disciplinary team and other agencies as
appropriate.

Undertake ordering and receipt of stores.

Undertake further training to gain skills in other areas.

Assist in the set-up of the Pulmonary Rehabilitation class

Support patients with the prescribed Pulmonary Rehabilitation exercises under the direct supervision of the Physiotherapist.

Provide refreshments to the Pulmonary Rehabilitation patients.

Support Administration staff in contacting patients for Pulmonary Rehabilitation and chest Physiotherapy.

To support with the delivery and collection of Respiratory equipment.
