Training and Development Continue with training and development to improve knowledge and skills pertaining to your role Confidentiality Maintain patient confidentiality at all times Be aware of the Data Protection Act 1998 and Caldicott guidelines Communications and Relationships Communicate effectively with patients and carers, recognising patients rights and responsibilities Work as a member of the Practice clinical team, seeking advice and guidance and support from other members Communicate concerns identified in patient contact to a member of the Clinical Team Work under the supervision of the Practice Nurse who will ensure that skills are maintained and developed to meet the needs of the post Be aware of, and adhere to, all Practice policies and procedures, especially: Health and Safety and Fire Regulations Equal Opportunities Disciplinary and Grievance Bullying and Harassment Decisions and Judgments Work at all times within the established policies, standards, and guidelines of the Practice Undertake clearly defined clinical duties which have been agreed and taught Refer back to a member of the Clinical Team any patient where an informed clinical judgement is required before proceeding with treatment Clinical Duties Observe infection control procedures at all times to protect self, patients and colleagues Assist in maintaining a safe working environment for self, patients, carers and the multi-disciplinary team Alert team members of issues of quality and risks identified in work with patients Work within established systems of clinical governance, contributing to practice audit activity Carry out health assessments on newly registered patients, testing urine, blood pressure, height, weight, and documenting medical history Act as a chaperone for patients as required, preserving the patient's dignity Prepare and stock clinical areas Order promotional materials and other stock when needed Wash and sterilise instruments in line with Practice protocols Provide relevant health information to patients and carers within defined protocols Refer patients back to other members of the clinical team any patient that doesn't fit within established protocols Undertake specified clinical activities and test for names patients, within the scope of Practice clinical protocols, policies, and procedures: Venepuncture ECG Urinalysis Routine BP Health Promotion Peak Flow Wound Care Attend relevant meetings as required Record Keeping Maintain accurate records of patient contact, providing clear written and verbal information as appropriate All patient contact must be recorded in the electronic records Additional Responsibilities The employer can determine any additional duties that maybe required so long as they do not exceed the level of responsibility expected for this grade of post.
