Working in Intermediate Care is an exciting opportunity that you may never have thought about before !
Intermediate Care is a vital part of health and social care, where you will support people who have experienced a period of ill health or injury to regain their independence and rehabilitate them back to doing the things that are important to them. That could be anything from improving a person’s walking, supporting someone to learn how to use a new piece of equipment, increasing a person’s independence getting washed and dressed to providing emotional support and encouragement when they need it most. No two days are the same!
We have 59 beds across two Intermediate Care Units based in Ellesmere Port Hospital. It is our aim to deliver person centred care, in the right place, at the right time.
We are currently looking for Healthcare Assistants to join our amazing team, working directly with Nurses, Occupational Therapists and Physiotherapists and therapy assistants in a fast-paced environment. Within the role you will also support people to attend exercise groups and reminiscence classes, accompany therapists on home visits and have freedom to bring your ideas forward on how we can deliver the best care possible.

1. At the direction of the Registered Nurse/Midwife participate fully in the delivery of planned nursing/therapy care.
2. Provide information to enable the effectiveness of care to be evaluated and records maintained.
3. Involve patients and carers/relatives in the delivery of care and provide reassurance support to patients and carers/family.
4. Work collaboratively with others on the Ward to ensure that patient’s needs are met.
5. Recognise and respond appropriately to urgent and emergency situations.
6. Provide escort and transfer for patients, with low dependency, as directed byRegistered Nurse/Midwife.
7. Ensure the effective and efficient use of resources.
The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.
The Trust provides services in all of the surgical and medical specialties common to a District General Hospital with some specialist services. The Trust serves the whole of the local population currently in excess of 250,000 residents which geographically covers populations in England and North Wales.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £23 (standard) or £43 (enhanced), this cost will be deducted from your salary over the first three months of employment.
You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
