Mind in Bradford is looking for a health engagement officer to expand our severe mental illness (SMI) and physical health programme
The candidate will play a key role in upskilling the approach of primary care staff in engaging and delivering physical health checks to people with SMI. The candidate will provide tailored outreach support to individuals on the SMI register, to increase engagement and uptake of physical health checks and associated physical health interventions.

Salary scale: £25, 596.66 per annum increasing to £27,186.12 on completion of a
satisfactory probation period.
Contract: Fixed term until 31st March 2025
Hours: 37.5 hours
Base: Kenburgh House, 28 Manor Row, Bradford, BD1 4QU and various GP
practices across Bradford
Responsible to: Programme manager
Annual Leave: 25 days pro-rata plus bank holidays
Pension: We operate a contributory pension scheme which you will be auto-
enrolled into (subject to the conditions of the scheme).
Probationary period: 6 months
