Job description
The post holder will provide health improvement expertise through delivery of structured Healthy Living Programmes for people at risk or diagnosed with long term condition. Specifically, the post holder will be supporting delivery of NHS Long Term Plan work remit including: Ageing Well plan, helping older people manage long-term conditions, making sure they receive the right kind of support to help them live as well as possible. Working alongside primary, specialist care, physical, MH services and social care services. Health and well-being, self-management and psych-education workshop for patients with long term condition Working alongside NHS Diabetes programme and self-management programme Use of Motivation Interviewing and other behaviour changes strategies to increase patients skills and confidence to manage their own condition and set achievable goals, and engaging patients in making informed decisions.
Weigh management, physical activity, smoking cessation and alcohol awareness support Support with mental and social wellbeing by signposting/referring. Delivery of Health Checks Connect with appropriate stakeholders (health professionals / community groups etc.) to target programme and its awareness, including supporting awareness raising events. Develop appropriate up-to-date resources
