We are looking for passionate and driven people who want to join our Oxfordshire-based team to work within the local community. You will work as part of a team of passionate health coaches and advisors as we launch a brand-new community outreach NHS Health Checks service across Oxfordshire.
You will deliver comprehensive NHS Health Checks to members of the public aged 40-74 years to provide a detailed assessment of their current state of health and to refer them and signpost them to relevant services to improve their health and wellbeing post check.
You will deliver checks in community venues and workplaces.
This role will work to agreed targets and objectives as part of the team to ensure that the service delivers what it needs.
The role is ideally suited to people with strong communication skills and those who are really passionate about making a difference to people’s lives on a daily basis.
It also includes working with vulnerable adults and such the successful candidate will be required to apply for a DBS prior to starting the role
We have a 3 days per week role.
Candidates MUST be able to drive to clinics .

Are you right for the job?
- You will need to be an independent self-starter, flexible and driven by engagement, outcomes and service management.
- You will be used to working towards outcome targets.
- You will live in the local to Oxfordshire area to be able to travel and work daily in Oxfordshire.
- You will own a car and be able to drive to client sites and other offices if required.
What you will do
- Deliver comprehensive NHS Health Checks including understanding lifestyle and health behaviours.
- Conduct bio metric testing including Blood pressure checks, cholesterol and HBA1c testing using Point of care testing kits (POCT) and provide clients with an assessment and results of their current health and level of cardiovascular risk.
- Use coaching techniques to ensure that the client understands the level of risk and discuss how changes the client can make including referring them to wider service support including our own Stop for Life smoking service, weight management services, physical activity interventions and the National Diabetes Prevention Programme.
- Manage the referral of your clients to their own GP for further clinical assessment depending on their bio metric results using the NHS Health Check digital tool.
- Be responsible for the accurate case management of each of your clients including accurate and robust data input onto the digital NHS Health Check tool.
- Manage your diary and time in collaboration with the Admin Co-ordinator and Senior Leadership team to ensure that you maximise capacity and support clients aligned to your individual targets.
- Monitor your own performance against your monthly performance targets to ensure you are contributing and supporting the success of the service and your wider team.
- Keep up to date with latest national guidance on delivering NHS Health Checks effectively and share any learnings with your team mates.
- You will be a Self-starter who enjoys working with people and helping them to understand their current health and most importantly encourage them to make changes to help improve their health and wellbeing over time.

- You will be a people-person, an enthusiast for how healthy lifestyles can impact positively on people’s health, someone who is fantastic at building rapport with an ability to persuade.
- You are willing to get out into the community and workplaces to develop relationships and set up clinics. You will be a self-starter, rather than waiting to be told what to do, eager to understand how the community works.
- You are resilient and driven by the desire to support people to be the best they can be.
- Full training will be provided during on-boarding.
- Experience in using Zoom, Microsoft Teams and other digital platforms.
