Mental Health Services for Older People are recruiting enthusiastic people who want to work witholder peoplewho have arange of mental health conditions.
You will haveexperience of working within health and/or social care providing direct personal care.
The Day Hospital services provide holistic mental health assessments, short-term therapeutic interventions, educational support and signposting services to older people with mental health issues. You will work alongside MHSOP secondary mental health teams to support the qualified nursing staff in providing enhanced assessment and support of older people.
The Day Hospital runs over five days (Monday to Friday) and you will rotate between the Day Hospital at Llandough Hospital and Grand Avenue Day Centre.
You will be part of amulti-disciplinary teamand your role will be to carry outperson centred careandactivitiesas designated by a qualified staff member. Activities may be 1:1 or group work and will be related toactivities of daily livingand/ortherapeutic interventions. These may take place within the Day Hospital or a community setting, therefore you will be confident to workwithout direct supervisionand you will be able totravelacross locations within Cardiff and the Vale of Glamorgan.
- Provide direct care and support to a defined group of service users, with varying needs.
- Close liaison with team members, Multi Disciplinary teams, Local Authority and voluntary sector, including feeding back outcomes of work and escalating any Concerns.
- Co-ordination and practical problem solving in relation to issues identified through assessment and intervention processes, including seeking appropriate support when urgent issues.
- Prompt, encourage and support people to participate in activities programmes and achieve goals identified in their care plans, whilst maintaining safety at all times.
- To document relevant information in relevant case notes from own observationIassessment and interventions, and as directed by clinician in charge.
- Contribute to the ongoing assessment and review processes, actively participating in discussions and bringing appropriate information to client.
- To establish links and communicate effectively with a variety of agencies, e.g. Alzheimer's society, social workers, CPN's, voluntary sector, MDTs, Community Mental Health Teams etc.
