NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included. This post will be part Home First Response service and will be based within Renfrewshire HSCP. The Home First Response service provides a MDT approach to support patients identified with frailty within the RAH and Community and work with MDT approach to support their discharge from hospital or prevent their admission. The successful candidate will have experience within a care environment, have or working towards SVQ level 3 or have equivalent experience and be able to work effectively within a community rehabilitation MDT team. The post holder will provide hands on care and support within a rehabilitation ethos. This post offers the opportunity to develop competencies across all health fields to be able to support nursing and AHP care plans, treatments and therapies as well as the opportunity to work closely with patients to enable them to live safely at home. Hours of work will be flexible to cover between 8.30 am and 9.30pm, 7 days per week.
