Heath Care Support Worker- Full/Part time Hours Community Mental Health Teams provide a service for individuals with complex mental health, physical and social care needs. As a multidisciplinary team we aim to provide care and treatment in the least restrictive manner possible, supporting individuals to lead a good quality of life that matters to them. The focus of this job for a Health Care Support Worker (HCSW)is to provide additional health and social care support inputs as part of the CMHT, which will involve working closely with a variety of disciplines including medical, nursing, social work, occupational therapy and psychology. The post-holder will be expected to take part in a wide range of therapeutic activities as directed by a qualified member of staff and as identified in care plans demonstrating the ability to work on their own initiative. Duties will include engagement; monitoring of treatment compliance through observation and assessment of the patients progress and assistance to maintain and organize daily routine including the transportation of clients as required. The post-holder will be expected to manage a range of duties delegated to them including
participating in assessment procedures, accurate record keeping, and participation in MDT meetings and active involvement of the patient and their care in these interventions. The post holder will be expected to participate in supervision and appraisal as an integral part of this post.
