Health Care Support Worker - Band 3
Five Rivers Health Centre
37.5 hours per week - Permanent

Here at Wiltshire Health and Care we are very excited to announce the development and expansion of our community services in line with the NHS Long Term Plan and a number of strategic priorities for community health services to transform our out of hospital care.
We are boosting the response capacity within the existing community teams to provide a dedicated resource across each of our localities within Wiltshire.
This growth within our services incorporates a consistent 2-hour crisis response element to our community services with appropriate skill mix of staff to support our current services across 12 hours (08:00 to 20:00) 7 days a week. We are also extending our services to provide nursing cover 24/7 in the community by developing an overnight element which will provide a much-needed service to our patients across Wiltshire.
