Job description

A new opportunity has arisen to develop wider highly skilled Band 4 team , who is resilient and resourceful individual's and could contribute to the transition and development of the reformed Mental Health Crisis pathways.

You will gain substantial experience in the variety of crisis response services working together to provide quick and highly skilled interventions to individuals experiencing and presenting with a variety of Serious Mental Illness (SMIs) in mental health crisis.

You will gain the experience of working with individuals presenting through different stages of crisis presentations; from the crisis line and NHS111 transfers. You will have the ability to identify those who need an urgent assessment and have the opportunity, to offer these individuals an assessment within 4-24hours.

You will have the opportunity to work alongside and within the Mental Health Liaison Team and Crisis Resolution and Home Treatment Team in working towards developing a cohesive crisis service pathway and supporting staff's professional development, experience, skills and knowledge. You will also be supported to develop skills through training provided by the trust to build assessment and formulation skills, risk management of psychotic, depressive, self-harm and suicidal presentation.

The is role is 24hrs , 7 days a week which includes night shifts and weekend on a rotation.

The post holder will support the team in prevent admission to a mental health hospital or to facilitate early and safe discharge from the ward. The post holder will be expected to visit service users in their homes or in community locations. The role will entail providing personal, practical and social support to service users, their families and carers using the principles of recovery, this may also involve supporting service users in our new and established crisis assessment centre, which currently houses our 24hr crisis line and support individuals who attend the crisis hub and A&E.
