Job description
We're looking for Healthcare Assistants to work directly with our patients, following direct instructions from professionals to help improve their health and quality of life. Our organization provides a wide range of health-care services and provides our patients the best possible care. The right individual for this role will take initiative to provide added value to our patients' care and provide support to the medical team when needed.
Responsibilities:
- Monitor the health and develop plans for patients with serious illnesses and complex needs, including those with mental illness or addictions, who are being treated in the home.
- Conduct medical screening interviews and complete admission assessments, as well as assessments for people moving out of hospital.
- Gather, record and communicate information about a patient's condition to members of the healthcare team.
- Participate in treatment planning and implementation of care plans, including medication administration and patient education.
- Administer medication.
- Provide emotional support to patients and their families.
- Manage all required forms, documentation and records.
- Perform other duties as requested.

Part-time hours: 20-40 per week
Salary: £13.00-£14.00 per hour
