Adur Community PCN Team A full time post has become available in our team and we would encourage anyone who is interested in a career in Community Nursing to apply. Our Senior Team are fully supportive of any new staff member who wishes to develop not only in the HCA role but also on to further education such as Apprenticeships at B3 and 5 level, Nursing Associate training and the various opportunities within the role. We are based at the Quadrant in Lancing and cover patients registered to any Lancing and Shoreham GP surgeries. Visits are allocated in geographical areas to ensure we are effective and efficient in our ability to see the patients on our caseload. You would join an established team of Registered Nurses, Nurse Associates and Health Care Assistants working closely with other SCFT Teams and our Primary Care Partners, GP's, Pharmacists, Therapists, Mental Health and Social Services including voluntary sector. We all work to provide support and treatments to those who are housebound in the community. This covers a range of health issues of which Wound Management, Diabetes Management, Dementia related issues, Bladder and Bowel and Palliative Care . To support the Registered Nurses to ensure the right care plans are followed for each patient. Training will be given to gain competencies in administering insulin to stable patients, also administering fragmin (anticoagulant) Wound care and leg ulcer compression bandaging, Continence management including catheterisation of stable non complicated catheters. Be able to carry out all duties as assigned by a registered nurse. Ability to assess at a basic level pertaining to the expectations of the role.
