Would you like the opportunity to work with GP practices? Join our successful and friendly team! 3 Health and Well-being Coaches (HWBCs) are needed to join the team.
Permanent contract - Full time, Mon -Fri, 35 hours per week (Fridays are mostly work from home).
Salary is £26,250 pa (Probation period - first 6 months it’s £25,650 after this it will increase).
The HWBCs will be employed by Brent Mencap and work as part of a Brent Primary Care Network (PCN) to promote and facilitate the development of a person-centred approach to behavioural and life style change. You will act as a guide to patients in addressing health issues and the need to improve health.
Many patients are living with conditions such as diabetes, heart disease and asthma and mental health issues so getting help from our local health services is more important than ever. That’s why many GP practices are working with each other and with community mental health, social care, pharmacy, hospitals, and voluntary services in their local area. These are called primary care networks. Working together means they can have a bigger teams, sharing information and proving better services to patients.
The post holder will work with a diverse range of people from different cultural and social backgrounds. They must have the ability to work confidently and effectively. Good communication is essential.
As a HWBC you will:
Act as a guide to patients in addressing health issues and the need to improve health.
Work with the your allocated PCN practices to development the PCN health improvement plan.
Provide education and training to patients, supporting them to improve their skills and understanding, behavioural approaches and ensuring consistency in the follow up of goals.
Support the PCN practices with uptake on screenings such as cancer screenings, breast and bowl screenings. Patients sometime miss appointments or not keen on doing these screenings as they are not aware of the importance. The HWBC will educate them, inform them of the importance and book in these appointments.
Coaching and motivating patients through sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.
Connecting patients to community-based activities and support groups so patients can take increased control of their health and wellbeing.
Increasing patient motivation to self-manage and adopt healthy behaviours.
