Derby City Council and Derbyshire Community Health Services have recently agreed to integrate a range of services that support people to remain at home when their needs change, but also to return home following a hospital admission by receiving rehabilitation and reablement support. Both organisations have been committed to working together for a number of years and we already deliver multi-professional assessments, are flexible in how we use our workforce and we share good practice on training and development. We are now pushing forward to achieve a greater level of “joined up” care for individuals and are looking for a highly ambitious and experienced manager who can help us take our vision forward and shape our new service offer – Community First.
This post is a unique position in that it will work to create and lead a unified health and social care team to provide seamless support for local people with the aim of keeping them living as independent as possible, and with good health and mental wellbeing. This includes ensuring that local people access reablement and rehabilitation support that maximises their own skills and abilities, even after a hospital or medical episode.
The postholder will be responsible for a pooled budget, agreed system level outcomes and a large multidisciplinary team, including allied health professionals, care workers and social workers. You will be an excellent and experienced leader, with a good track record of transformation and operational delivery as well as being able to work across a wide range of networks, not just limited to health and social care partners, but including the voluntary and community sectors and housing organisations.
On behalf of the Derby City health and care partnership, you will be expected to lead your teams to:
- Ensure more people are supported with their urgent health and care needs at home
- Ensure across Derby, there is consistent care offer with more people supported to be able to manage for longer at home.
- Reduce the referral points and processes that individuals and their families have to navigate so they only have to “tell it once”.
- Ensure the new Community First team unified, high performing and able to respond flexibly to urgent demands for health and care services.
- Harness the strengths of a wide range of professional staff to deliver the required service outcomes and increase the productivity of our teams.
This post is offered on a permanent basis and there is flexibility available for the successful applicant to be employed by either Derby City Council or Derbyshire Community Health Services. The post involves hybrid working but candidates must be available to work flexibly within the geography of Derby and Derbyshire.
