We are currently looking for an enthusiastic, highly motivated individual to work in the Early Supported Discharge Team for Stroke.
This high profile community based service covers Mansfield, Ashfield, Newark and Sherwood and receives referrals for patients following a stroke from both acute and rehabilitation hospitals. This multidisciplinary team was established in 2009 and has forged close links with local hospitals.
The team has a strong ethos of patient centred care and high clinical standards. The service aims to facilitate a smooth and timely transition from hospital to home optimising stroke recovery, function and independence.
We are looking for someone to join the team in the role of Generic Rehabilitation Support Worker assisting the qualified therapists and nurse in the ongoing rehabilitation of stroke patients accepted onto the scheme. You would be required to provide patients with rehabilitation based on their needs including physiotherapy, occupational therapy, speech and language therapy, nursing and aspects of clinical psychology. This is an exciting and fulfilling role.
- To provide a timely patient-centred, specialist stroke rehabilitation service responsive to individual need based in the community.
- To provide high quality service as part of a dedicated specialist stroke rehabilitation service with the local communities of Mansfield and Ashfield and Newark and Sherwood localities.
- Maximise independence and active lifestyle and minimise disability.
- Being proactive in facilitating independent living in the community wherever possible, by working with patients and their carers and other members of the team by setting realistic goals and evaluating their outcomes.
- To liaise with other health and social care services facilitating the provision of a seamless service for the patient.
- Training and education of physiotherapy, occupational therapy, medical, nursing and other professional staff/students as required.

Provide support to the senior Physiotherapy, Occupational Therapy and Speech and Language Therapist clinicians and Nurse colleagues in the ongoing rehabilitation of stroke patients accepted onto the Stroke Early Supported Discharge scheme within the community setting.
To contribute to a high quality therapy service, by assisting qualified senior staff in the implementation of treatment programmes and evaluation in conjunction with other members of the MDT. Also taking a responsibility in assessing within level of competence in the community setting.
Support patients/clients to improve or maintain their level of function and quality of life and advise how they can continue this in their own environment.
To promote an understanding of healthy lifestyle in the wider health community and in Social Care, and to promote an understanding of when patients need to access therapy services and when other health and social care services may be required.
