About the Trust – Rated ‘Good’ in their most recent CQC report, Greater Manchester Mental Health NHS Foundation Trust (GMMH) provides inpatient and community-based mental health care for Bolton, Manchester, Salford, Trafford and Wigan. The Role:
The successful applicant will assist in the delivery of care to patients with complex physical healthcare needs at the Edenfield Centre.
You will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will at all times comply with the NMC Code of Conduct, relevant legislation policies and procedures.
Healthcare services operate 7 days a week, 24 hours a day, and the successful applicants would need to be flexible in their approach to meeting the demands of the service
What you’ll be responsible for
- The post holder will undertake risk assessments, devise written plans of care, liaise with and refer to, any appropriate individual, agency or service that can meet the patient’s needs.
- Assess, plan prioritise, implement and evaluate nursing interventions to meet patient needs.
- Contribute to the organisation of multi professional, patient centred care. Enable patients to take an active role in care given.
- Undertake practice sensitive to the needs of patients/clients from multi cultural backgrounds. Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.
- Recognise the positive effects, and side effects of prescribed drugs and drug interactions. Administer medication in line with NMC guidelines.
- Be actively engaged in, and co-ordinate the discharge of the patient under the supervision of, and in co-operation with, other members of the multi disciplinary team
- Demonstrate optimum skills, in the establishment and maintenance of effective systems of communication with all disciplines and agencies.
- Ensure high standards of documentation are maintained, on all aspects of care, ensuring that records are contemporaneous
- Communicate appropriate information to patients’ relatives as agreed by the patient and the clinical team in line with Trust policies, and Caldicott guidelines
- Contribute to service development as appropriate. Respond to emergency calls, assess patients’ condition in an emergency situation and provide appropriate care and referral.
- Deliver effective, safe and high standards of care that it is within clinical competences. Adhere to all security regulations within the service.
You’ll have the following qualification/experience:
**ILS PMVA BLS can be provided by Trust if necessary** plus induction
· A minimum of 6 months paid experience in an Acute hospital environment within the last 3 years, experience in Adult Forensics desirable.
· Comply with NMC Professional Registration and keep up to date with current nursing practices
· Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
· Undertake regular mandatory training*
*Depending on the Trust, you may require variable additional certification and skills.
