
General Practitioner Responsible for Virtual Ward London, England
Job description
The post holder will be responsible for the delivery of high-quality services from the Virtual Ward based at North Middlesex University Hospital and bedded units at Cape Town and Canterbury Wards (Chase Farm Hospital Site)
The GP will deliver care of the highest clinical standards and will work in conjunction with the consultants, Service Manager, and the nursing team to ensure effective overall performance of the outlined services.
- To triage referrals from consultants, doctors, and specialist nurses across NMUH.
- To provide complete consultations with assessment, treatment and management of adult patients that are pre-medically optimised.
- To record full clinical notes using the NMUH IT systems (care flow).
- To complete coding requirements on the clinical record in line with the Trust and North Middlesex University Hospital requirements.
- To complete all admin work relating to consultations.

As with other ICS’s, we are working increasingly closely with partners and indeed many of our financial and performance objectives are measured at this system level. Whilst all organisations remain as standalone, statutory bodies we have an ICS infrastructure for making shared decisions and agreeing shared approaches.

- To work as part of the wider clinical team- supporting VW nurses, HCAs, and other clinicians on the wards.
- To visit VW patients in their homes for review.
- To conduct daily onsite ward rounds on Cape Town and Canterbury Wards.
- To support the redirection of patients to their registered GP where appropriate on discharge.
- To give education to patients on healthy living and disease prevention.
- To give education to patients on appropriate use of NHS services
- To prescribe medicines as a result of tests carried out with individual patients
- To complete discharge letters for all patients seen in the VW
- To adopt a reflective approach to clinical practice, to report all incidents and work with the team to investigate incidents
- To prepare responses for any complaints concerning his / her work
- To follow the Trust safeguarding vulnerable adults’ policy
- To restrict the use of diagnostics according to the specification of VW
- To maintain full registration with the GMC and have full medical liability insurance
- Actively contribute to clinical governance and clinical audit activity.
- Attend MDT meetings and other meetings as necessary
