Overview
The Delamere Medical Practice is excited to advertise a totally flexible new role for a General Practitioner providing services for our cohort of Discharge to Assess and Challenging Behavioural Unit patients across Trafford.
Visits can be organized throughout the week, on weekends or as needed.
Duties
- Visit various care homes in the Trafford area to assess and prescribe medication for patients.
- Organize and communicate referral request with dedicated admin team.
- Answer dedicated mobile and deal with urgent queries, prescriptions and request from various care homes within Trafford area.
- Organize visiting days for care homes and communicate patient medical needs and prescriptions.
- Travel to various care homes at least once a week or as needed based on your medical knowledge of patient.
- Maintain procedural structure in place to operate efficient.
Additional
- Ensure accurate computerised record taking of all consultations including telephone messages and visits.
- Comply with requirements of Practice Based Commissioning
- Comply with practice prescribing formulary/guidelines

- Provide constructive criticism to streamline service
Person Specification
Experience
Essential
Completion of Vocational training
Commitment to professional/peer group learning
Desirable
Experience of working in a GP Practice.
Experience of working in a Care Home.
Essential
Full registration with GMC and a recognised medical defence organisation.
Must also be registered and approved on the Medical Performers List
Pre-employment checks all SGPs will be required to undergo an enhanced DBS check prior to being approved to join the practice.
Qualifications
Essential
Post graduate qualification in General Practice
Completion of GP vocational training

Excellent clinical, communication and interpersonal skills
Willingness to contribute to multi-professional working
Ability to work well autonomously and with a team.
Evidence of accountability
Desirable
Knowledge of IT Clinical Systems (EMIS,)
Understanding of the Principles of clinical audit
