Job description
The Forensic Community Social Work Team is currently undergoing a new re-modelling of service as part of the Thames Valley Forensic Mental Health service initiative to enhance the quality of care it provides to service users in their own homes. The new service will become a Specialist Community Forensic Team (SCFT) once the re-modelling process is complete.
As a result, we are looking for three highly motivated Social Work professionals who are keen to utilize their skills, and experience, to bring the social care perspective into the forensic mental health community setting, to support new and existing service users in the team in their recovery journey. The posts will be suitable for experienced candidates who are resilient, enjoy working as part of a multi – disciplinary team, and are sensitive to the needs of mental health service users, and their relatives in the community.
These are permanent positions and will be based at Oxford Clinic Littlemore Mental Health Centre site in Oxford and Marlborough House in Milton Keynes (base of Buckinghamshire staff).
This post attracts a RSU Lead of £1,446 per annum on top of your annual salary.
- To carry out the role of Care Co-ordinator to an allocated number of service users and take responsibility for ensuring that individual care is assessed, planned, and reviewed as part of the Care Programme Approach.
- To provide a social care service to an allocated number of service users and their carers who are accepted by the Forensic service.
- To initiate and maintain good links with local authorities of the area of origin of Extra Contractual Referral patients (ECR) within the service.
- To have direct contact with service users via casework and input to a clinical treatment programme. This will include undertaking social circumstances report, Mental Health review Tribunal reports, Managers Hearing report and section 117 Aftercare responsibilities.
- To ensure that all service users in their caseload have a full assessment of their Care and Support needs under the Care Act 2014 and that support planning takes place to meet these needs.
