Overview

We are seeking to appoint a Consultant with complementary skills to the local and regional requirements. This offers an excellent opportunity for someone with the appropriate subspecialist skills to join the team to cover a potential catchment area of our client’s NHS facility.

Applicants must hold full registration and a licence to practice with the GMC.

Candidates for Consultant posts must also be on the GMC Specialist Register (including via CESR/European Community Rights) or will have a CCT/CESR(CP) date within 6 months of interview.

Essex is a county in the East of England, north-east of London. One of the home counties, it borders Suffolk and Cambridgeshire to the north, Hertfordshire to the west, Kent across the estuary of the River Thames to the south and the London boroughs of Enfield, Waltham Forest, Redbridge and Havering to the south and south-west. The county town is Chelmsford, the only city in the county. For the purposes of government statistics, Essex is placed in the East of England region.

There are four definitions of the extent of Essex, the widest being the ancient county. Next largest is the former postal county, followed by the ceremonial county with the smallest being the administrative county – the area administered by the County Council, which excludes the two unitary authorities of Thurrock and Southend-on-Sea and the areas administered by the Greater London Authority.

The ceremonial county occupies the eastern part of what was, during the Early Middle Ages, the Kingdom of Essex. As well as rural areas, the county also includes London Stansted Airport, the new towns of Basildon and Harlow, Lakeside Shopping Centre, the port of Tilbury and the borough of Southend-on-Sea.

A high proportion of the population, especially in the south, work outside the county, commuting to London and elsewhere by rail and by road. These London-based jobs are often well paid and complement the contribution made by the employers based within Essex.

Industry is largely limited to the south of the county, with the majority of the land elsewhere being given over to agriculture. Harlow is a centre for electronics, science and pharmaceutical companies.

Much of Essex lies within the London commuter belt, with radial transport links to the capital an important part of the area's economy. There are nationally or regionally important ports and airports and these also rely on the Essex infrastructure, causing an additional load on the local road and rail links.

The main airport in Essex is Stansted Airport, serving destinations in Europe, North Africa and Asia. London Southend Airport, once one of Britain's busiest airports, opened a new runway extension, terminal building and railway station in March 2012. It has a station on the Shenfield to Southend Line, with a direct link to London.

Department

The inpatient rehabilitation service provide care to patients with severe mental illness; primary diagnosis of schizophrenia, schizoaffective disorder and bipolar disorder with comorbid diagnoses. These inpatient rehabilitation units provide treatment for chronic and recurrent presentations, manage risk, facilitate recovery for patients and plan a safe, effective discharge into the community.

The Trustwide service comprises of male High Dependency Rehabilitation Units (x2), female High Dependency Rehabilitation Units (x1); Complex Care Wards (x2); Community / Move On Rehabilitation Units (x 3). These were developed to create a trust wide service In addition to the above; community rehabilitation is also available in some localities through a community rehabilitation team or through the Step-up Hub.

General Duties- guideline only

The successful applicant’s primary role will be as a Consultant Psychiatrist providing senior medical input into a dedicated ward. This is a high dependency male ward and provides service to all of Essex localities. The successful applicant will join three other rehabilitation consultants in the service working on the F Ward (female High Dependency Unit), another consultant on M Ward (move on unit), the third consultant who works on C Ward (Complex care unit). Consultant cross cover is provided between the rehabilitation consultants on the site for annual leave and study leave.


There are 3 Acute adult wards; 2 male and 1 female; 1 PICU (mixed gender); 1 male HDU, 1 female HDU and 1 complex care rehabilitation ward on the site. All the wards have 18 beds except PICU which has 14 beds. The move on unit is 10 beds and is based in the service.


The post holder will work with the ward manager, clinical leads, nurse consultant and other service colleagues as part of the leadership team to provide care and improve the care for patients. The post will involve providing medical leadership and input to the multidisciplinary teams at board reviews, care coordination reviews and weekly multidisciplinary review meetings.


An 18 bed unit. Currently there are 17 patients and all are detained under the Mental Health Act. The average length of stay ranges between 12-24 months. In the last 12 months there have been 10 discharges to community and 3 transfers into other inpatient services.


Referrals to the ward are generally received from Adult acute inpatient units, PICU, forensic units and out of area services to repatriate patients. The post also involves working with the wider rehabilitation team to undertake assessments of patients referred to the rehabilitation services through the assessment clinics, attending the weekly referral meeting and helping to manage the care pathway of patients through the rehabilitation units. It also includes assessing patients referred from Out of Area placements who may be suitable for rehabilitation. This role is not onerous and is shared between ward managers and the senior medical team within the rehabilitation services. The post holder will provide senior expertise in the assessment, treatment and rehabilitation of patients admitted to the service, as well as providing assessment and advice to colleagues working on the acute inpatient wards.


There are regular leadership meetings on each ward across the site. The ward has external links with local voluntary groups and third sector providers, for example with local dog charities. Promoting social inclusion and reducing stigma are two of the aims of these groups. Service users are encouraged to participate in activities and attend Recover Colleges.


Job Plan

Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.


The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide and will be finalised following individual agreement with successful candidates. Further discussion on supporting professional activities will depend on the Trust's requirements and the individual's particular expertise.

Salary


This is as described in the Medical and Dental Terms and Conditions, in line with the Consultant Contract (2003). The current full time salary scale (as of July 2020) ranges from £84,559 - £114,003 a year with eight thresholds.


The on call supplement is Category A and attracts a supplement of 5% of basic salary.

Office Accommodation

Office accommodation will be provided for the appointee with secretarial support.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the existing clinical team through, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.

Teaching/Research

The consultant will be required to participate in programmes for teaching clinical students, training junior doctors and in clinical examinations. The post-holder’s contribution to teaching, training and research will be included in the regular job plan review. Teaching will take place during programmed activities and flexibly at other times.

  • To provide conditions for improved training opportunities in line with national and local recommendations arising out of GMC and related requirements
  • To supervise and manage the work of his/her junior medical staff and participate in the training and development of these staff to the standards required by the Royal Colleges.

Clinical Governance & Audit

The consultant will participate in clinical governance activities, including clinical audit, clinical effectiveness, risk management, quality improvement activities as required by the Trust, and external accrediting bodies. There are 6 half day clinical governance meetings per annum which substitute regular fixed sessions. There is a weekly consultant meeting before MDT on Friday to discuss any issues during the week and to plan for next week.

Personal & Professional Development

The post-holder will be required to keep himself/herself fully up-to-date with their relevant area of practice and be able to demonstrate this to the satisfaction of the Trust.

The Trust supports the requirements for continuing professional development as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities.

Medical revalidation is the process by which the General Medical Council (GMC) confirms the continuation of a doctor’s licence to practise in the UK, provides greater assurance to patients, the public, employers and other healthcare professionals that licensed doctors are up-to-date and fit to practise. It is a key component of a range of measures designed to improve the quality of care for patients.

The Trust supports Royal College guidance on provision of mentors for new consultants in line with GMC recommendations. All newly appointed consultant staff are provided with a list of locally trained mentors who are available for ongoing support as required.
