
Discharge Support Worker London, England
Job description
We are looking for a highly motivated, well organised, enthusiastic individual to work flexibly as a full time 37.5 hours/ week Discharge Facilitator within Surgery.
You will be responsible for improving the patient’s experience and supporting effective, timely and safe discharges by means of providing practical support to the multidisciplinary team and nurse in charge in accordance with departmental and Trust policies and procedures.
You will be expected to have excellent communication and organisational skills and the ability to work autonomously as part of a multi-disciplinary team.
