JOB TITLE: Digital and Transformation Lead
RESPONSIBLE TO: Clinical Director for PCN
ACCOUNTABLE TO: Islington GP Federation
PLACE OF WORK: Agreed locations in Islington
HOURS: 37.5 per week
SALARY: Band 8a (£55,903 - £61,996 based on experience)
CONTRACT TYPE: 12 month fixed-term, with possibility of extension
The Role:
The post-holder is expected to be an experienced digital lead with considerable transformation experience, preferably within a primary care environment. The post-holder will help practices and the PCN adopt new technology and other initiatives to improve access to care, enable practices and PCN staff to work more effectively, coordinate across digital platforms and critically enable patients to self-serve.
The post holder will work with the Clinical Director, Practice Managers and the leadership within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise and leadership to deliver changes in a collaborative way.
This role will include delivery of a combination of the following responsibilities:
- Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity.
- Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access).
- Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving.
- Review and improve the PCN’s digital maturity.
Use data, and improve data quality, to:
- Understand demand, capacity and activity and drive improvements in:
1. Patient experience of access.
2. Operational efficiency including better matching capacity to need.
3. Staff experience at work.
- Support population health management.
The post holder will work closely with staff working at practices, such as practice managers and staff at the PCN to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
The post holder will work at least four (4) days at agreed PCN locations in Islington.
Key requirements for the Successful Candidate:
- Experience of working in general practice with a good working knowledge of general practice systems (including IT, digital and BI tools), processes, and procedures; orexperience of successfully delivering change and improvement programmes in a patient (or customer) facing environment.
- A track record of communicating and engaging with a wide range of staff and stakeholders; building excellent and trusted relationships.
- Experience of using programme management and improvement techniques to drive change forward, including translating strategic objectives into tangible plans for delivery and following these through to benefits realisation.
- Ability to extract, collate, visualise and use data and insight to both drive change, and to understand and adapt programmes of work while in progress to allow learning to be integrated into delivery.
- Knowledge of Quality Improvement tools or a willingness to engage in the Quality Improvement training offers available to support the Digital and Transformation Lead role.
- Ability to understand, interpret and present complex and/or sensitive information.
- Ability to interpret qualitative and quantitative information from a range of sources and present it in a clear and concise way to stakeholders.
- Excellent communication skills and an ability to engage successfully with a wide range of people at all levels within an organisation
- Experience of championing diversity and inclusion and promoting actions to make improvements to the experience of diverse groups, including promoting digital inclusion.
- An ongoing commitment to improving NHS services for the benefit of patients.
About South PCN and Background
The South Primary Care Network (PCN) is a group of 7 independent GP practices in the centre of Islington, covering a population of around 74,000 residents. The PCN is brought together by NHS England’s PCN DES contract to deliver against its specifications and other tasks. The PCN works with multiple stakeholders as part of the South Locality, a multi-agency group focused on identifying population health needs and identifying streams of work to support these. This is part of Islington’s ‘Fairer Together’ borough partnership.
The population within the PCN geography is diverse, often with significant and complex health and wellbeing needs. There is an exciting opportunity for enthusiastic and motivated individuals to support the population health and working closely with people to support them to achieve improved health outcomes and a better quality of life.
As this is a new role and the guidance around PCNs is constantly changing, there is a need for the postholder to be flexible and adaptable in this evolving environment. As a consequence, the post will be subject to ongoing review and inevitable iteration and will help inform how roles like these can be effectively used to support patient care.
About Islington GP Federation
Islington GP Federation (IGPF) is a growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islington’s extended access primary care services (I:HUB) as well as supporting the Islington Primary Care Networks (PCNs). Our current range of services include the Extended Access Service, I:HUB, Community Ear, Nose and Throat (ENT), Integrated Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms.
IGPF works very closely with a range of partners including the regional commissioning group, NHS England, Healthy London Partnership, Public Health, local hospitals such as UCLH and the Whittington Health and the London Borough of Islington.
IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington Training Hub, and has been working for over two years to create training and development programmes that meet the needs of staff working in primary and community care settings. IGPF runs two Islington GP practices on caretaking contracts; a third practice will be taken on within this financial quarter.
Job Responsibilities
Quality and Service Improvements and Responsibilities:
The role will require experience in quality / service improvement with knowledge in transformational change and a good understanding of general practice systems and processes, including familiarity with general practice digital systems such as EMIS, Docman and how they can be used to deliver improvement.
This expertise will be used to:
- Work alongside practices to understand current challenges to effective service delivery and identify solutions, including existing best practice within the PCN.
- Drive the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in the practices within the PCN.
- Build relationships with practices across the PCN to develop consensus for improvements and to ensure all staff (clinical and non-clinical) are fully engaged throughout the process of selection, implementation, deployment and monitoring/evaluation of change, peer challenge and learning.
- Develop and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering the potential impacts on the wider organization, and determining resource requirements.
- Provide hands-on capacity and support to practices in implementing and sustaining agreed changes.
- Support staff to identify and participate in relevant training and/or professional development opportunities that will support achievement of agreed improvements, and optimisation of existing and new technology.
- Advise on innovative opportunities and support practices to maximise benefits from innovation, through the sharing of learning and best practice.
Information and Data Management:
The post holder will need to have skills in relation to understanding and using data to drive improvement and assess impacts. These skills will be used to:
- Take a data driven approach using a variety of sources and looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.
- Bring together data from a variety of sources and use information to develop insights into working practices, or support population health management, risk stratification or workforce planning, effectively tailoring content to meet the needs of the audience
- Use data to understand demand and capacity, and to drive improvements of patient experience
- Support and encourage a focus on data in understanding the effectiveness of changes made
- Seek and secure analytical resource as required to support the PCN in developing and implementing appropriate data and information sharing mechanisms
- Use data to track and assess impact of changes made to operational processes, and to track trends and developments over time.
Communication and Engagement:
With responsibility for developing and supporting delivery of transformational projects across the PCN, the post holder will initially need to be build strong connections with the practices in PCNS by bringing people together and gaining support and buy-in to change.
These skills will be used to:
- Ensure that changes are aligned with agreed clinical priorities and have ownership and engagement from clinical leaders and staff of the PCN
- Build and maintain good working relationships with a broad range of internal and external stakeholders, Initiate and participate in development of effective systems of communication and co-operation between practices, staff, and other members of the primary care team
- Support practices to communicate changes and new processes effectively and consistently to patients.
- Identify best practice and learning from other areas that could be applicable to the local PCN and communicate this to staff.
- Make connections across the PCN to support learning between practices and sharing of best practice.
- Lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN (and ICS) to agree changes, share learning, discuss challenges, and resolves issues.
