Job description
CLINICAL Be professionally and legally accountable for own work and caseload working to the allocated job plan and job map for the role. Work as part of the MDT to provide a comprehensive paediatric diabetes service to children attending the Childrens hospital as in-patients, day cases or outpatients. Maintain strong channels of communicationto include external agencies as needed (e.g educational settings, social workers, other MDT professionals across the County) To provide timely dietetic support to newly diagnosed type 1 diabetic patients travelling to the Horton hospital where required and prioritising caseloads. Be up to date with menus and catering across sites to support carbohydrate counting and ensure that age appropriate, nutritionally adequate meals are available within the Childrens Hospital that tailor for therapeutic diets.
Judge and determine appropriateness of referrals made by members of the multidisciplinary team to ensure effective use of time and resources. Prioritise own workload according to clinical need, managing the caseload effectively and efficiently. To be aware of own limitations and ask for support where needed. To be able to work autonomously when needed and manage a specialised caseload.
Adhere to the OUH clinical governance frameworks including Safeguarding children and vulnerable adults and attend safeguarding and multiagency meetings where required. To have a strong foundation in nutritional support to include enteral feeding (including home enteral feeding in line with OUH policies) Have a sound knowledge of feeds, formulas and special diet products and the use of EDM 3000 for feed recipes. To considerprescribing guidelines and feed budgets when considering feed or formula choices and onward prescriptions. To inform, advise and teach patients, carers and health professionals on the nutritional and dietary requirements and individual care of patients using a range of communication skills and tools.
This involves being able to communicate complex and sensitive information to a wide range of people, and requires highly developed negotiation, motivation, counselling and inter-personal skills. To be able to address barriers to communication e.g. use of interpreters, visual aids. To independently assess nutritional status based on anthropometric measurements e.g.
weight, height, weight history as well as biochemical parameters, diet history and other validated methods of nutritional assessment as appropriate. To formulate dietetic interventions and care plansthat are practical and evidence based and evaluate efficacy, communicating any changes to caregivers and staff. Support safe discharges by timely handovers and ensuring a clear dietetic care plan for home is available. To be responsible for maintaining accurate and comprehensive electronic patient records in line with legal requirements and Trust guidelines.
NON-CLINICAL Demonstrate a commitment to learning and embracing new technologies and digital innovation. To be professionally and legally accountable for all aspects of your own work, including sole responsibility for the dietetic management of patients in your care, working within the professional, ethical, and legal framework for dietetics set out in the Health Profession Councils Dietitians Standards of Conduct Performance and Ethics and the British Dietetic Association Professional Standards for Dietitians and Code of Professional Conduct. Participation in education and CPD within the Paediatricdiabetes MDT and department. To be an active part of the paediatric dietetic team, providing clinical cover and supervision when needed to meet the needs of both the team and service users.
To participate in the department bank holiday on call. To use audit, quality improvement and other tools to evaluate service provision and support service and business case development. To educate, train and disseminate information to healthcare professionals including clinicians, nurses, allied health professionals, dietetic colleagues, and students where appropriate. To participate in department meetings To support digital innovation and telemedicine for the paediatric dietetic service.
To lead on resource development as needed by the specialist service in line with OUH diet sheet regulations. To maintain an up-to-date portfolio in line with HCPC requirements to demonstrate professional competency.
