At ULHT we have the privilege of providing the Home Enteral Nutrition service for the whole of Lincolnshire on behalf of the ICS.

We currently work along side our commercial partners Fresenius Kabi.

To date we provide a HEN service to 743 patients across Lincolnshire.

The Community HEN service looks after Paediatric , Neuro Outreach, Oncology, Tertiary referral In reach: like GOSH, Adult Stroke, Nursing Home Older Adults and Learning Disabilities.

This role will be supported by the Band 7 Head & Neck Oncology Dietitian who already has oversight of 245 Oncology patients.

This role will compliment a department with 3 Band 6 HEN Dietitians and 2,6 Band 3 Dietetic Assistants.

We have just invested in the latest IT and provide the option for patients to have Telephone, Virtual or Face to Face consultations.

This is a PAN Trust role, although the current staff are based out of Lincoln County Hospital.

The service will provide support to patients receiving enteral nutrition at home.

To provide a specialist, cost efficient and high quality nutrition and dietetic service to Home Enteral Nutrition within ULHT, working closely others involved in the care of these patients and in accordance with NICE and other national guidance.

To provide advice on the appropriate dietary manipulation and management of patients receiving enteral nutrition in the community setting.

To contribute to, initiate and develop Trust policies and procedures relevant to the patient demographic evidencing based best practice.

To maximise the nutritional status of patients in the caseload.

Ensure that all patients under the care of the home enteral nutrition team receive an appropriate level of service

To provide training on relevant dietetic topics for patients, carers and staff members.

To ensure all areas of the service are covered appropriately during periods of leave.

To contribute to the development and evaluation of the post.

To be compliant with all relevant national standards related to the specialities home enteral nutrition and other areas relevant to the caseload

To provide a high standard of clinical care ensuring safe and effective assessment of nutritional diagnosis, taking into account medical condition, environment and lifestyle using highly developed clinical reasoning skills. To work in partnership with the patients and carers to develop, implement and review nutritional plans based on a complex analysis of clinical nutritional aspects, including biochemistry, anthropometry, the patient’s clinical profile and considering the patient’s distress and the financial implications of the plan. Devise means to evaluate progress towards goals.

Take specific responsibility for the assessment, development of specialist treatment plans and evaluation of patients with complex needs including those who may have multiple pathologies. Organising and prioritising designated caseload effectively and efficiently.

To attend relevant MDT meetings and case conferences.

To provide supportive and sensitive communication, written and verbal advice to patients, carers and staff, whilst demonstrating an understanding of the nature and effects of some information and of barriers to effective communication e.g. sensory impairment and unrealistic expectations.

Demonstrate empathy and understanding of social, psychological, ethnic and religious factors which effect compliance

Following the completion of competency-based training, company feed pumps and the r.

To liaise with and provide expert opinion on dietetic issues directly contributing to the care of patients, to medical, nursing staff and other members of the MDT.

To provide specialist advice to Dietetic colleagues working in other clinical areas with regards to the care of patients within this speciality

To provide specialist nutrition and dietetic input into education, training and development programmes for undergraduate and postgraduate, medical, nursing and other students To provide spontaneous and planned training, instruction and advice to patients and carer/relatives regarding their nutritional needs.

Identify the training needs for a wide range of health care professionals e.g. dietitians, doctors, nursing staff and other healthcare staff. Organise and provide the training as required.

To provide clinical leadership in the speciality of home enteral nutrition.

To be responsible for own administration tasks and to co-ordinate and monitor those of the team including prompt and accurate completion of electronic records, clinical and statistical records and performance monitoring following HCPC, BDA, Trust and departmental guidelines.

To adhere to the standards, policies and procedures of the Trust and Department and

the BDA, HCPC Code of Ethics and Professional Conduct and associated legislation.

To supervise the clinical practice of members of the team ensuring high standards of clinical care. To undertake and provide clinical supervision sessions according to local policy

To work collaboratively with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the Trust and with external agencies

Develop local and Trust standards, protocols and policies for nutritional care in enterally fed patients in the community using evidence based practice.

To represent the Dietetic Service at relevant speciality meetings at Trust level.

To confidently convey complex written and verbal information on both clinical and non-clinical issues and to support the development of these skills with individuals within the team.

Responsible for complying with the hygiene code and ensuring all working practice supports infection prevention and control including awareness of hand hygiene and environmental cleanliness in all clinical areas.

To demonstrate empowering leadership skills within the Dietetic service and seek opportunities in local and national arenas to promote and develop the profession.

To provide high quality practice placement education for Dietetic students and apprentices.

To identify own training and development needs and those of the team including the development of competencies. Active participation in the delivery of training opportunities for staff and students, including those from other teams and disciplines

To maintain own continuing professional development and use reflective practice to evaluate and update current practice. To promote a learning environment and culture within the clinical team.

To evaluate, plan and lead service development in conjunction with other team leads and the Site Lead Dietitians within the relevant clinical specialities and with colleagues.

To undertake governance activities, including, induction, supervision, appraisal, negotiating personal development plans, mandatory training, health and safety, risk management and audit and manage this within the clinical team.

To contribute to the recruitment and selection of Dietetic staff

To contribute to departmental routine including housekeeping and clerical tasks.

To produce or review diet sheets and nutritional resources as required, in line with current guidelines and evidence based practice.

To undertake specific project work or any other duties related to the specialty as negotiated with the Site Lead Dietitians PAN Trust or Trust Lead Dietitian.

Ensure a cost-effective service, paying close attention to the relative costs of products supplied to patients, and discouraging and preventing wasteful practices.
